Proxy access was develop to support patients who require additional support from another Indvidual when accessing or managing their online GP information.
In order to create a proxy access account, you will need to complete the proxy access form and return this into the surgery with a photo ID (Passport, driving license etc). Once the completed form has been returned we will assess your application and if appropriate make changes within the clinical system.
Access may not always be granted if the surgery believe that the application is not within the best interest of the patient.
