Registering With Us

Joining our list is very simple, if you wish to look around or ask any questions before you register please call in and see the receptionist.

You may register at either the Sabden or Whalley Surgery.

Please bring your medical card (Med 4) into the Surgery. There should be a card for each member of your family. This card has your name, address, NHS number and the name of your previous Doctor on it. We then send this card to the Lancashire and South Cumbria Agency, who then apply to your previous Doctor for your medical records. If you do not have, or cannot find, your medical card then we have a form GMS1 for you to complete.

If you have moved to a rural area we may be able to dispense medication to you from the Surgery. You will be advised if you come from a dispensing area by the staff.

All newly registered patients are invited to attend the Surgery for a short consultation with either one of the partners or Practice Nurse. If you take regular medication we would ask you to make an appointment with one of the Doctors, and if you are not on medication, the Practice Nurse. The purpose of this consultation is to assist us, both in assessing your previous medical history, and in your overall future medical care.

We are a fully computerised practice, and will make entries regarding vaccinations, cervical smears etc. so that you can be placed onto a recall system. Please bring any relevant information along with you. We ask that you bring a urine sample to the Surgery when you attend this consultation (containers available at reception).

Children under the age of 5 years are not requested to attend this consultation, but their names will be added to the Child Health Screening programme at the Surgery. Their names will be passed to one of the Health Visitors attached to the Practice, who will make contact with you should this be appropriate.

You will be given a Practice booklet showing the services available at the Surgery. It contains details of Surgery hours, home visits, appointments, repeat prescriptions etc. and introduces you to the Practice.

You will be registered with one of the partners, however we do not insist that you see that particular Doctor. Patients are free to see whichever Doctor they are most comfortable with.

Patients wishing to see a Doctor out of Surgery hours are advised to telephone the out of hours service onĀ 111.

There is a copy of the Practice Charter on display in the waiting areas of both surgeries.
